A SWAN Units of Service meeting for Post-permanency will be held on December 11, 2025, from 10:00 a.m. to 12:00 p.m.
This session will present a case example using the newly updated Respite Plan and answer any questions related to the updated Respite Plan. Expectations for Respite purchased services documentation will be discussed.
