Description

A SWAN Units of Service meeting for Post-permanency will be held on September 24, 2025, from 10:00 a.m. to 12:00 p.m.

This session will present updated criteria and documents for the Post-permanency services. Attendees will be guided through a review of the updated Reauthorization Criteria for determining family eligibility for additional services. Additionally, participants will be introduced to the new Respite Plan and how to complete the updated components as well as the new Support Group Information Form and how it will be implemented.
